Understanding Your Coverage
For services covered by your in-network plan, you will typically be responsible for any co-payments, co-insurance, or deductibles at the time of service. Our team will assist you in understanding your estimated financial responsibility.
Self-Pay & Wellness Services
Certain medical services such as prolotherapy, platelet-rich plasma (PRP), bone marrow aspirate concentrate medicinal signaling cells (BMAC), and shockwave therapy (ESWT / EPAT) are not covered by insurance.
Many of our advanced Wellness Clinic services, including certain Functional Medicine consultations, IV Nutrient Therapy, and aesthetic treatments, may not be covered by insurance. We offer transparent self-pay pricing for these services. Please contact our office for details on specific consultation and treatment fees. We are happy to provide you with an itemized receipt (a “superbill”) that you may be able to submit to your insurance for potential out-of-network reimbursement, depending on your plan.
