What Happens to Your Nervous System in a Stuck Activated State
People who’ve experienced ongoing trauma, especially in childhood, often develop a significantly smaller window of tolerance.[3] Their nervous system has less capacity to regulate, which keeps them cycling between hyperarousal and hypoarousal with very little room in between.
Genuine calm gets misinterpreted as unsafe. It’s like asking someone mid-panic attack to just calm down. Without context for safety, the nervous system won’t allow it.
In this state, being alone becomes loneliness instead of solitude. Excitement gets perceived as fear. You’re perpetually waiting for the other shoe to drop, even when nothing is actually wrong. So the nervous system stays on guard, all the time, and that vigilance becomes the new baseline.

Work becomes a form of flight response. Volunteering becomes an escape from what’s truly needed. From a polyvagal perspective, these are still rooted in sympathetic nervous system activation (fight, flight, fawn), all oriented toward avoiding dorsal vagal shutdown. That state of depletion, helplessness, and hopelessness feels even more threatening than the exhaustion itself.
These patients often present with chronic pain, physical and mental fatigue, brain fog, and empathy fatigue. There’s typically self-criticism, people-pleasing tendencies, poor self-compassion, and difficulty setting limits. They tie self-worth to being accepted and helping others, without any intrinsic self-love as the foundation.
Chronic hyperarousal isn’t benign. Your body wasn’t designed to sustain this level of activation. It contributes to chronic pain, high blood pressure, cardiovascular problems, and increased risk for Type 2 diabetes.[5]

I never dismiss pain. As a physician, I always rule out structural causes first. But I also look for compensatory patterns, habitual guarding, and pain that responds to non-physical factors.
Consider social context. I work with people whose physical pain increases during emotional turmoil: arguments with partners, friction at work, family tension. That same person, sitting and watching TV on vacation versus sitting in a work meeting, experiences completely different pain levels. Same position. Different context. Different pain.
This is well-documented in pain research. Exaggerated psychological responses to stressors intensify sympathetic and neuroendocrine activity, which perpetuates widespread pain and inflammation.[6] The environment perceived safety shapes how much pain the nervous system generates.
The body doesn’t lie. When pain shows up in calm moments or peaceful positions, it’s telling you that the relationship with stress has become unsustainable.

The real goal is expanding your window of tolerance to stress, adversity, pain, discomfort, and uncertainty. That’s what builds genuine resilience. Research confirms that people with childhood trauma often have smaller windows, spending most of their time in states of hyper or hypoarousal.[3]
The work starts with psychoeducation. Understanding what’s happening in your nervous system removes shame from the equation. You’re not weak. You’re wired this way for a reason.
Then comes pattern recognition: identifying the specific factors and states that aggravate your pain experience, building comfort with discomfort, reducing dependency on certainty, practicing gratitude and self-compassion, and setting limits instead of pushing through overwhelm.
This process works against established neural networks. It’s like trying to change a deeply familiar handshake. We don’t fight the old pattern directly. We build new habits that run parallel to the old ones but lead to different destinations. Over time, the old pathways prune, and neuroplasticity reinforces the new ones.
There’s a phase I prepare every patient for: the extinction burst. Things get worse before they get better.[7] This is where most people mistake progress for failure and revert to what feels familiar.
