Recent 2025 research shows that approximately 30% of the population scores high on sensory processing sensitivity (SPS).[1] That’s nearly one in three people.

SPS is not a disorder. It’s a temperament trait, a fundamental difference in how the nervous system processes information.

Highly sensitive people (HSPs) don’t just feel more. They process more deeply.[2] Their brains show heightened activity in regions involved in awareness, empathy, and sensory integration. The key player is the insula.

Bottom line: If you’re an HSP, your nervous system isn’t malfunctioning. It’s working as designed, just with a much higher gain setting.

How the Insula Amplifies Pain in HSPs

The insula sits deep in the brain. It’s the integration hub for everything happening inside your body.[3] It processes sensations from your organs, muscles, and tissues. It tracks your heartbeat, your breathing, the tension in your shoulders.

For HSPs, the insula shows heightened activity when processing sensory information and emotional cues.[4] This means HSPs have a more active interoceptive system. They’re more aware of what’s happening inside their bodies.

In many ways, that’s a gift. Deeper awareness. Stronger intuition. Richer inner life.

But when pain enters the picture, that same awareness becomes a liability.

What Is Bidirectional Sensitization?

This is where most doctors miss what’s actually happening.

HSPs don’t experience pain more intensely because they’re focused on it. Their nervous systems are wired to process sensory information more deeply. When pain signals arrive, the insula amplifies them. The brain reads these signals as more threatening, more urgent, more dangerous than they are.

And it doesn’t stop there.

Chronic pain changes the brain. Research shows that persistent pain causes gray matter alterations in pain processing networks, including the prefrontal regions, thalamus, cingulate cortex, anterior insula, and basal ganglia.[5,6] The very regions already hyperactive in HSPs become even more sensitized.

This is what I call bidirectional sensitization: heightened sensitivity amplifies pain signals, and chronic pain further sensitizes the nervous system. Each reinforces the other. It’s a feedback loop, not a character flaw.

Key Point: Bidirectional sensitization means you’re not stuck because you’re weak. You’re stuck because two biological systems are reinforcing each other. That’s a solvable problem.

Why Doctors Get It Wrong: The “Catastrophizing” Label

This is where the system fails HSPs most completely.

When an HSP describes their pain in vivid, precise language, “It feels like shards of glass moving through my muscles” or “It’s a burning, electric sensation radiating down my entire leg,” doctors often reach for the “pain catastrophizing” label.[7]

Research tells a damning story here. About 45% of chronic pain patients have heard this term. About 32% consider it problematic. Twelve percent report being labeled a “pain catastrophizer” by a clinician, and most describe the experience as feeling blamed, judged, and dismissed.[7]

HSPs aren’t catastrophizing. They’re accurately reporting what they feel.

Their heightened interoception means they perceive subtle variations in sensation that others miss. They’re not exaggerating. They’re describing their actual neurobiological experience. The problem isn’t their perception. It’s a medical system that doesn’t recognize different nervous systems process pain differently.

This dismissal does real damage. It invalidates the patient’s experience. It reframes a neurobiological reality as a psychological failing. It implies they’re the problem.

And it closes the door on appropriate treatment.

Central Sensitization: When the Volume Dial Gets Stuck

When I work with HSPs in chronic pain, I frequently see central sensitization: a state where the nervous system amplifies pain signals and lowers the threshold for what triggers pain.

Think of it as your nervous system’s volume dial getting stuck on high. Sensations that shouldn’t be painful become painful. Mild discomfort becomes severe. The system meant to protect you starts sounding alarms constantly, even when there’s no real emergency.

For HSPs, this process happens more readily. Their already-sensitive nervous systems are primed for this kind of amplification. The genetic factors behind high sensitivity, particularly involving dopamine and serotonin, overlap with those involved in central sensitization.

But here’s something important: not every HSP develops chronic pain, and not everyone with chronic pain is an HSP.

The relationship is complex. Environment matters. Stress matters. Trauma matters. Sleep matters. Social support matters.

HSPs are more vulnerable under stress, sleep deprivation, or trauma. Their nervous systems are more responsive to both negative and positive environmental influences. This is differential susceptibility: the same trait that creates vulnerability in harsh environments creates flourishing in supportive ones.

The Allostatic Load Problem: Why the Body Keeps the Score

Another piece of this puzzle is allostatic load, the cumulative burden of chronic stress on the body.

Your nervous system is constantly working to maintain balance. When you face a stressor, your body mobilizes. Heart rate goes up. Cortisol rises. Muscles tense. This is adaptive in the short term.

But when stressors pile up, work pressure, relationship difficulties, financial strain, poor sleep, past trauma, the system never fully recovers. The load accumulates.

For HSPs, accumulation happens faster. Their nervous systems are more reactive to environmental stimuli. They pick up on subtle cues others miss. They process experiences more deeply. All of this costs energy and creates physiological demand.

When allostatic load exceeds the nervous system’s capacity to regulate, pain becomes more likely. The system gets stuck in a protective state, interpreting normal sensations as threats.

What Actually Helps: A Different Approach for HSPs

Standard pain management often falls short for HSPs. You need something built around how your nervous system actually works.

1. Start with validation.

Your pain is real. Your detailed descriptions aren’t catastrophizing, they’re accurate interoception. You’re not broken or weak or “too sensitive.” Your nervous system is doing exactly what it’s wired to do. It’s doing it in overdrive.

2. Reframe your relationship with interoception.

The goal isn’t to stop noticing what’s happening in your body. That’s both impossible and counterproductive. The goal is to change your relationship with those sensations. To notice them without immediately reading them as dangerous. To observe without amplifying.

This takes practice. It takes patience. It requires learning to separate sensation from threat.

3. Address allostatic load directly.

You can’t treat the pain in isolation. You have to look at everything keeping your nervous system in chronic activation: sleep, stress, relationships, boundaries, past trauma, inflammation. All of it matters.

Reducing allostatic load gives your nervous system the capacity to recalibrate. To turn down the volume dial. To stop reading every sensation as an emergency.

4. Titrate everything.

HSPs need smaller doses of everything, movement, exposure to triggers, even positive experiences. What works for others might overwhelm your system. Start smaller than you think you need to. Build gradually. Respect your nervous system’s need for slower integration.

5. Use objective data to rebuild trust.

Years of being told you’re “too sensitive” can erode confidence in your own perceptions. Objective measures help. Heart rate variability. Sleep tracking. Pain patterns over time. Data builds trust in your experience and helps you spot patterns that aren’t visible in the moment.

Key Point: Treatment for HSPs with chronic pain isn’t about toughening up. It’s about working with the nervous system you have, not fighting against it.

The Strength Side of Sensitivity

Here’s what often gets lost: sensitivity isn’t only a vulnerability. It’s also a strength.

Research shows that while some components of SPS correlate with anxiety, aesthetic sensitivity associates with positive outcomes. HSPs tend to show higher receptivity to positive events, stronger intuition, deeper empathy, and greater responsiveness to social cues.

The same nervous system that makes you vulnerable to pain also makes you more responsive to healing. More receptive to supportive relationships. More capable of deep processing and insight.

This is vantage sensitivity, the flip side of vulnerability. In the right environment, with the right support, HSPs don’t just recover. They thrive.

This takes time. It takes patience. It takes a willingness to work with your sensitivity rather than fight it.
